Freedom Health Group believes to be an informed and responsible health care consumer you need accurate information on cost. With Freedom Health Group, there are no hidden charges, no surprises at the end of your visit with the health care provider, and no surprise bill at the end of the month.

Full Membership

If you are an individual, a single parent, a family with children, or a Senior, then a Full membership is the best program for you. The Full membership allows members unlimited $25 office visits and blood lab tests that range from $20-$70 each. A Full membership can help you save money in managing your chronic health issues that need the supervision of a health provider. A Full membership gives parents an option for getting their children seen for ear infections, sport injuries, vaccinations, and cuts and bruises at an affordable cost. Memberships for children are always free with a parent's membership. If you are a Senior without a primary care provider, then a Full membership will give you the care that you need.

We also offer a Limited Membership option designed for individuals, allowing them access to an annual physical and two office visits per year for only $250.00.

Membership Program Costs

*Members are granted access and receive health advocate services upon Freedom Health Group receiving and processing the member’s payment and completed application.

Right to Cancel
Members may cancel within the first 30 days. After 30 days the annual access fee is not refundable. If the member cancels within the first 30 days they understand and agree they are responsible for a $30 processing fee, the cost of any office visits during the initial 30 days charged at the non-member rate, and billed by the provider.
