Freedom Health Group believes to be an informed and responsible health care consumer you need accurate information on cost. With Freedom Health Group, there are no hidden charges, no surprises at the end of your visit with the health care provider, and no surprise bill at the end of the month.

Membership – Simple and Affordable

For individuals, single parents, a family with children, Seniors, and employers wanting an affordable health benefit for their employees.

Memberships for children are free with a parent's membership.

Membership Program Costs

• Children under the age of 18 living with parent(s) receive free membership access when parent(s) pay for a Membership.

*Members are granted access to Direct Care program and receive health advocate services upon Freedom Health Group receiving and processing the member’s payment and completed application.

Access to Low Cost Lab Test Fees and Vaccination

Lab tests and vaccinations are at a reasonable price. Below is a selected list of the more common lab tests a member might be required to have at the health care provider’s direction.

Supplemental Accident Insurance

Freedom Health Group has teamed up with Unum to offer our members another benefit they could not get by themselves as Unum does not offer this program to individuals. This accident insurance benefit can help members pay for medical expenses they may incur as a result of an accident that happens while not at work. It helps with hospital expenses, urgent care, and office visit expenses resulting from an accident that happens at home, on vacation, or otherwise away from the job. There is no additional cost to the member. Members with children who would like to purchase the supplemental accident benefit for their children can for just $14.95 per month.
There are exclusions that include, but not limited to: acts of war, bungee jumping, parachuting, fall from a hot-air balloon, to name a few.

Right to Cancel
Members may cancel within the first 30 days. After 30 days the annual access fee is not refundable. If the member cancels within the first 30 days they understand and agree they are responsible for a $75 processing fee
