Applying for a Freedom Health Group membership for yourself or your family is easy. Simply download the Application Form PDF below, print out the application, complete and sign the form, then mail it to the address below with the total membership fee due. Learn more about our Membership level options. You can also drop off the application and fee at our office located at 13607 E. Sprague Avenue in the Spokane Valley.

After your application and membership fee is received, the application process will take approximately three to five days to complete. At that time you will be
free to call and immediately schedule your first appointment or walk-in if you
have an urgent care need.
